If you think that you may require vaccinations relating to foreign travel please check the links below before you contact the surgery. These will give you general advice about the areas you are travelling to and will also inform you if vaccination is recommended depending on where you are visiting and the type of accommodation you are staying in.
If you do require vaccinations please complete the travel questionnaire form via the link below or ask for a form at reception in the surgery. Please ensure you complete the form at least six weeks before travel, and be aware that you will need an appointment with the nurse to discuss your requirements and order the necessary medications.
Some vaccinations are supplied under the NHS and others are supplied privately and incur a charge. Requests for private travel immunisations with less than six weeks notice may mean that we are unable to supply the immunisations at the surgery and you will need to contact a Travel Clinic. The nearest to the surgery is either the Salisbury Travel Clinic 01722 331191 or the Bath and Bristol Travel Clinic 01225 820461. You will need to telephone the clinic for an appointment.
On receipt of the completed form the receptionist will arrange a telephone consultation with Nurse Polly who is our Travel Nurse. Polly works at the surgery on a Tuesday and Friday and she will then contact you at the agreed time by telephone to discuss your requirements and make any necessary appointments.
